PATIENT APPOINTMENTS

Dr. Galbraith, Dr. Schmidt and Dr. Abdo-Stone are pleased that you have taken the time to visit our dental website. Our commitment is to serve our dental patients so that they may achieve quality oral health and well-being. The dental care that we provide is uniquely designed for each patient's needs to enhance their appearance, comfort, and function. At Galbraith Family Dental, you can be assured of receiving excellent oral health care consistent with your patient needs.

Our business office staff will assist you in scheduling your dental appointments, ensure necessary paperwork is completed for your insurance payments, and answer any billing questions that you may have.

Patients are seen by appointment only. We make every effort to be on time for our patients, and we ask the same courtesy from your family. If you cannot keep an appointment, please notify us immediately, giving a minimum of 24 business hours notice to avoid any missed appointment fees. We realize that unexpected illnesses and emergencies may occur, but we ask for your assistance in this regard.

INITIAL OFFICE VISIT

During your first visit, we will begin by reviewing your medical and dental history forms and taking xrays. Then one of our doctors will meet with you to discuss your xrays, chart existing treatment you have had completed previously, help create a treatment plan designed just for you and answer any questions you may have.

At your next visit, one of our hygienists will clean and polish your teeth, measure the health of your gums and inspect thoroughly for sign of decay or gingivitis.

After your initial set of appointments your exam and xrays will be scheduled together within the same appointment.

The American Dental Association recommends a visit to your dentist every six months for regular checkups and routine teeth cleanings to keep your teeth and gums healthy. If you choose to go further between cleanings your appointments may be scheduled differently. If it has been more than 6 months between cleanings, it can take multiple appointments to get you back on track.

If you have x-rays at another office please contact that office and ask them to email your records to info@fairbanks-galbraithdds.com. You can complete the form below titled "records release incoming" and submit it to your previous dentist. If you would like us to transfer your records please complete the record release outgoing form below.

DENTAL INSURANCE

If you have dental insurance that we will be billing for your appointment, please call us with that information prior to your appointment. We will need:

  • Your dental insurance company name & phone #

  • Your subscriber ID

  • Your group #

  • The policy holders name & birth date

  • The policy holders employer (if any)

This allows us time to contact your insurance company and obtain a breakdown of the benefits your employer or you have purchased from the company. In addition, you will want to bring a dental benefit card and/or book that your carrier should be able to provide. Our business team will be happy to help you understand your policy.
